Chief complaints : 44 year old male came to OPD with chief complaints of loss of appetite since 1 month , cough and expectoration since 10 days and sob since 3 months. I am G sravanthi of 3rd semister This is an E log book to discuss our patient health data Cheif complaints :  A 64 year old male came with a cheif complaints of pain in the right loin since 1 month and vomitings since 1 month . HOPI : patient was apparently asymptomatic 10 years ago ,then he developed pain in the right loin for which he diagnosed to be having renal stones and got  operated for it . After 3 years he developed bilateral loin pain and diagnosed to be having bilateral renal stones and got operated for it. After 3 years he had a midline swelling and got operated for it. Then after a year he got diffused abdominal pain he went to hospital and endoscopy was done and diagnosed it as intestinal ulcer and managed conservatively . At that time he was also diagnosed with hypertension and diabetes mellitus.  and since 1 month abdominal pain in right loin and vomitings. The patient’s consent was taken prior to history taking and examination of his condition. Cheif complaints: A 47 year old female which cheif complaints of pedal edema shortness of breath & abdominal distension and cervical lymphadnopathy  HISTORY OF PRESENT ILLNESS: patient was apparently asymptomatic 6 years back and had generalized body weakness for which she went to hospital and diagnosed it as diabetes mellitus 2 and was on regular medication . 2 months back patient had pedal edema below hip ,pitting type and abdominal distension for which she visited near by hospital and was diagnosed hypotension and hypothyroidism after necessary test pedal edema was subsided on medication but abdominal distension was not subsided for which she came to her hospital and was admitted and treate
